Reference Sheets: We require you have three reference sheets filled out and sent two weeks before your interview. Your pastor needs to complete one and non-family members must complete the other two.

Criminal report: This form must be filled out online with a credit card at https://epatch.state.pa.us. Please fill out as soon as possible; it take up to a month to process.

Childline Abuse History Clearance: Print and fill out Childline Abuse History Clearance. Send to the address on the top of the form with a money order; they do not accept checks.
